Your first visit
Thank you for choosing Atwood Family Medical Center! You will need to complete some extra paperwork prior to your first visit. Print out the forms further down this page and return them to our office or fax them to (757) 548-3665. Our staff will enter your data and contact you within 48 hours to schedule your first visit with our office. If you have any questions about the information on the forms or what to bring to your first visit, please call us at (757) 436-1234. Please be sure to bring your current insurance card, a picture ID and your complete medication list to every office visit.
