Atwood Family Medical Center has begun to receive SMALL shipments of the Moderna COVID-19 vaccine. The office will contact eligible patients based off the Virginia Department of Health guidelines to schedule a vaccine time during one of our clinics. Walk-ins will NOT be accepted. Please DO NOT CALL THE OFFICE to see when you will be able to receive the vaccine as our shipments vary from week to week.

You are required to attest that you have read the U.S. Food and Drug Administration (FDA) COVID Emergency Use Authorization (EUA) prior to being administered the COVID-19 vaccine. To facilitate our vaccination process, bring your completed Vaccine Screening Form, picture ID and insurance card(s) to your vaccine appointment.
