COVID-19 Vaccine Information

Atwood Family Medical Center has begun to receive SMALL shipments of the Moderna COVID-19 vaccine. The office will contact eligible patients based off the Virginia Department of Health guidelines to schedule a vaccine time during one of our clinics. Walk-ins will NOT be accepted. Please DO NOT CALL THE OFFICE to see when you will be able to receive the vaccine as our shipments vary from week to week.

After you have been scheduled to receive the Moderna COVID-19 vaccine, you will need to review the links below. You are required to attest that you have read the U.S. Food and Drug Administration (FDA) COVID Emergency Use Authorization (EUA) prior to being administered the COVID-19 vaccine. To facilitate our vaccination process, bring your completed Vaccine Screening Form, picture ID and insurance card(s) to your vaccine appointment.

A mask/face covering/shield IS REQUIRED to enter the Atwood Family Medical Center COVID-19 vaccine clinic. IF YOU ARE EXPERIENCING COVID-19 symptoms (see link below), you will need to reschedule your appointment until your symptoms have resolved.

  • Clinics will be held on the 2nd floor at Atwood Family Medical Center
  • Arrive 5 minutes before your scheduled appointment for your COVID-19 vaccination
  • If you arrive late to your appointment, you may have to wait or reschedule
  • Walks-ins for the COVID-19 vaccine will NOT be accepted
